Si se tiene una visi\u00f3n normal, la retina contiene fotoreceptores denominados conos y bastones que convierten la luz en impulsos el\u00e9ctricos que se transportan al cerebro a trav\u00e9s del nervio \u00f3ptico. Posteriormente el cerebro procesa los impulsos en forma de informaci\u00f3n visual de una manera similar a las fotograf\u00edas de una c\u00e1mara.<br \/>\nExisten varios tipos de afecciones que afectan a la retina pero lo habitual son da\u00f1os que se producen en el tejido de la retina, lo que ocasiona una interrupci\u00f3n en la transmisi\u00f3n de la informaci\u00f3n desde los fotoreceptores al cerebro. Si estos da\u00f1os no se detectan y no se tratan correctamente por parte de un especialista en oftalmolog\u00eda, se puede producir una p\u00e9rdida progresiva de la visi\u00f3n y, en ocasiones, ceguera.<\/p>\n<p>&nbsp;<\/p>\n<h4>\u00bfCu\u00e1les son las enfermedades de la retina que afectan a los reci\u00e9n nacidos?<\/h4>\n<p>Las enfermedades de la retina de los beb\u00e9s pueden ser muy distintas de las de los adultos, por eso existen algunos espectros especiales que han de tenerse en cuenta a la hora de tratar a pacientes pedi\u00e1tricos que padecen este tipo de afecciones. Estas mutaciones pueden ocasionar que las c\u00e9lulas sigan creciendo y se multipliquen en lugar de morirse, que es lo que hacen las c\u00e9lulas sanas. Esta masa acumulada de c\u00e9lulas forman un tumor.<br \/>\nCada c\u00e9lula suele tener dos genes\u00a0<a href=\"https:\/\/www.cancer.org\/cancer\/retinoblastoma\/causes-risks-prevention\/what-causes.html\" target=\"_blank\" rel=\"noopener nofollow\">supresores del tumor RB1<\/a>\u00a0que controlan la rapidez a la que crecen las c\u00e9lulas y hacen que las c\u00e9lulas mueran en el momento adecuado. Si funciona al menos un gen RB1, la c\u00e9lula de la retina no formar\u00e1 un retinoblastoma. Sin embargo, si mutan los dos genes RB1, la c\u00e9lula puede crecer de manera descontrolada y ocasionar m\u00e1s mutaciones en los genes, lo que puede ocasionar que las c\u00e9lulas se vuelvan cancerosas. La mutaci\u00f3n de RB1 puede producirse en todas las c\u00e9lulas del cuerpo o \u00fanicamente en una c\u00e9lula de un ojo:<sup>4<\/sup><\/p>\n<ul>\n<li><strong>Retinoblastoma hereditario:<\/strong>\u00a0<a href=\"https:\/\/www.cancer.org\/cancer\/retinoblastoma\/causes-risks-prevention\/what-causes.html\" target=\"_blank\" rel=\"noopener nofollow\">Aproximadamente 1 de cada 3 ni\u00f1os<\/a>\u00a0con retinoblastoma tienen una mutaci\u00f3n de la l\u00ednea germinal en un gen RB1 que produce la mutaci\u00f3n en todas las c\u00e9lulas del cuerpo. En el 75 % de los casos, la mutaci\u00f3n de RB1 apareci\u00f3 en una c\u00e9lula del \u00f3vulo o espermatozoide que form\u00f3 al ni\u00f1o o en una de las c\u00e9lulas del ni\u00f1o durante el embarazo. El 25 % restante de los ni\u00f1os desarrollan un retinoblastoma hereditario porque sus padres tambi\u00e9n han experimentado la mutaci\u00f3n de RB1 y se la han transmitido al ni\u00f1o.<sup>5<\/sup><\/li>\n<li><strong>Retinoblastoma no hereditario:<\/strong>\u00a0En la mayor\u00eda de los otros dos tercios de ni\u00f1os que padecen un retinoblastoma, la mutaci\u00f3n del gen RB1 se produce en los primeros d\u00edas de vida y al principio solo se produce en una c\u00e9lula de uno de los ojos. En este caso, la mutaci\u00f3n del gen no se trasmite de manera hereditaria.<sup>6<\/sup><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>S\u00edntomas del retinoblastoma<\/strong><br \/>\nUn retinoblastoma muchas veces se detecta porque el ojo del ni\u00f1o tiene un aspecto inusual. Normalmente, cuando la luz entra en el ojo, la retina refleja una peque\u00f1a cantidad de esa luz atr\u00e1s a trav\u00e9s de la pupila. Debido a los vasos sangu\u00edneos situados en la parte posterior del ojo, la luz reflejada tiene un color rojizo, que se conoce como reflejo rojo. Las anomal\u00edas en la retina pueden ocasionar una falta de reflejo rojo o un reflejo pupilar blanco, tambi\u00e9n denominado\u00a0<a href=\"https:\/\/www.cancer.org\/cancer\/retinoblastoma\/detection-diagnosis-staging\/signs-and-symptoms.html\" target=\"_blank\" rel=\"noopener nofollow\">leucocoria<\/a>.<sup>7<\/sup><\/p>\n<p>Otros signos y s\u00edntomas menos habituales de un retinoblastoma son<sup>8<\/sup>:<\/p>\n<ul>\n<li>Problemas de visi\u00f3n<\/li>\n<li>Dolor en el ojo<\/li>\n<li>Enrojecimiento de la parte blanca del ojo<\/li>\n<li>Sangrado en la parte delantera del ojo<\/li>\n<li>Protusi\u00f3n ocular<\/li>\n<li>Una pupila que no se hace m\u00e1s peque\u00f1a cuando se expone a luz brillante<\/li>\n<li>Un color diferente en cada iris<\/li>\n<\/ul>\n"}},{"acf_fc_layout":"right_image_and_left_content","_acfe_flexible_toggle":"","right_image_left_content_single_cta_landing":{"module_options":{"":null,"module_id":"n65291638bf4ee","module_class":"","module_background_type":"color","module_background_color":"","module_background_image":false,"module_background_video":"","activate_custom_padding":false,"padding_top_desktop":0,"padding_top_tablet":"","padding_top_mobile":"","padding_bottom_desktop":"","padding_bottom_tablet":"","padding_bottom_mobile":"","activate_custom_margin":false,"margin_top_desktop":"","margin_top_tablet":"","margin_top_mobile":"","margin_bottom_desktop":"","margin_bottom_tablet":"","margin_bottom_mobile":"","disable_on":[],"content_alignment_desktop":"left","content_alignment_tablet":"left","content_alignment_mobile":"left"},"":null,"label":"","title":"2. Retinopat\u00eda del prematuro ","description":"<p>La retinopat\u00eda del prematuro (ROP, por sus siglas en ingl\u00e9s) es una enfermedad de la retina que puede afectar a beb\u00e9s prematuros. La retina no se desarrolla por completo en estos beb\u00e9s y el desarrollo anormal de los vasos sangu\u00edneos en la retina d\u00e9bil puede generar problemas de visi\u00f3n.<br \/>\nSeg\u00fan el\u00a0<a href=\"https:\/\/www.nei.nih.gov\/learn-about-eye-health\/eye-conditions-and-diseases\/retinopathy-prematurity\" target=\"_blank\" rel=\"noopener nofollow\">National Eye Institute<\/a>, entre 14.000 y 16.000 beb\u00e9s se ven afectados por alg\u00fan grado de ROP cada a\u00f1o en los Estados Unidos. Aproximadamente el 90 % de estos casos se consideran leves y los beb\u00e9s no necesitan tratamiento\u00a0ya que los s\u00edntomas de la enfermedad desaparecen gradualmente.<\/p>\n<p>&nbsp;<\/p>\n<p>Sin embargo, en los casos\u00a0graves, a los beb\u00e9s se les puede desprender la retina (imagen 3), es decir, la retina se separa de la capa de los vasos sangu\u00edneos que le proporciona ox\u00edgeno y nutrientes, y, si no se trata, puede producirse ceguera. Las cataratas cong\u00e9nitas pueden darse en uno o ambos ojos y son las responsables de casi el\u00a0<a href=\"https:\/\/en.wikipedia.org\/wiki\/Congenital_cataract\" target=\"_blank\" rel=\"noopener nofollow\">10 % de todas las p\u00e9rdidas de visi\u00f3n que se producen en los ni\u00f1os en todo el mundo<\/a>.<sup>13<\/sup><\/li>\n<li>La\u00a0<strong>conjuntivitis neonatal<\/strong>\u00a0es una inflamaci\u00f3n del ojo externo que afecta a los beb\u00e9s tras su nacimiento. Normalmente se debe a una infecci\u00f3n bacteriana neonatal contra\u00edda durante el parto vaginal como consecuencia de la exposici\u00f3n a bacterias del canal uterino, como Neisseria gonorrhoeae o Chlamydia trachomatis. Por todo ello, se recomienda aplicar pomada antibi\u00f3tica a los ojos de todos los reci\u00e9n nacidos antes de que transcurra una hora desde su nacimiento para prevenir la conjuntivitis neonatal.<\/li>\n<li>La\u00a0<strong>toxoplasmosis<\/strong>\u00a0se debe a una infecci\u00f3n producida por el\u00a0<a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/toxoplasmosis\/symptoms-causes\/syc-20356249\" target=\"_blank\" rel=\"noopener nofollow\">par\u00e1sito toxoplasma gondii<\/a>, que suele ingerirse al comer carne contaminada poco cocida o exponerse a heces de gato infectadas. Un beb\u00e9 tiene m\u00e1s riesgo de contraer la toxoplasmosis si la madre se infecta en el tercer trimestre del embarazo.<sup>14<\/sup><\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong>S\u00edntomas de las enfermedades oculares cong\u00e9nitas\u00a0<\/strong><br \/>\n<a href=\"https:\/\/childrensnational.org\/visit\/conditions-and-treatments\/eye-conditions\/congenital-developmental-anomalies-affecting-the-eye-and-orbit\" target=\"_blank\" rel=\"noopener nofollow\">Algunos de los s\u00edntomas de las enfermedades oculares cong\u00e9nitas<\/a>\u00a0son un aspecto anormal de los ojos y una reducci\u00f3n de la visi\u00f3n. El BIO se coloca en la cabeza del oftalm\u00f3logo y permite ver el interior del ojo a trav\u00e9s de una lente de aumento. Permite observar la retina moviendo el BIO y la lente y aplicando depresi\u00f3n escleral.<sup>16<\/sup><\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>Tradicionalmente, este proceso llevaba mucho tiempo y obligaba al oftalm\u00f3logo a trasladarse al lugar donde se encontrara el paciente. La evaluaci\u00f3n puede realizarse de manera eficaz mediante el uso de im\u00e1genes de diagn\u00f3stico digitales del ojo en combinaci\u00f3n con la telemedicina.
